初次来我校的学生，需在申请居留许可前提供在有效期内的、由出入境检验检疫中心出具的《境外人员体格检查记录验证证明》。

Students who come to our school for the first time are required to provide the Certificate of Verification (Physical Examination Records For Foreigners or Overseas Chinese) issued by the Entry-Exit Inspection and Quarantine, the Public of China before applying for a residence permit. And the Certificate of Verification should be within the validity period.

若在常州，请前往常州市检验检疫中心（地址：江苏省常州市新北区龙锦路1268号B座）进行体检，需提前预约。

预约方式如下：

If you are in Changzhou, please go to Changzhou Inspection and Quarantine Center (Address: Block B, No. 1268 Longjin Road, Xinbei District, Changzhou City, Jiangsu Province) for physical examination, you need to make an online appointment in advance, as follows:

a. follow the official account named "常州国际旅行卫生保健中心"

b. tap "business processing"

c. choose "appointment"

d. choose "personal appointment"

e. choose "entry and exit health examination"

注意事项：

1. 体检前需空腹；

2. 需携带本人护照、2张2寸证件照及足量现金；

3. 如需取消预约，需提前24小时完成；24小时内取消或失约，将进入黑名单，当月无法再预约。

Note: 

1. Do not eat or drink before taking a physical examination.

2. Make sure to bring your passport, two 2-inch ID photos and sufficient amount of money (around 343 RMB) with you.

3. If you need to cancel the appointment, do it at least 24 hours before the scheduled time; if you cancel the appointment within 24 hour or break the appointment, you will be on the blacklist and you will not be able to make another appointment in same month.
