Documents You Must Have Before Applying a Residence Permit

如果你是老生，你必须持有：

If you are a returning student, you should have:

1. 有效期12个月以上的护照

Passports valid for 12 months or more.

2. 填写好并且粘贴好照片的签证申请表（照片必须为近一年内拍摄的2寸白底签证专用照片，否则请前往指定地点重新拍摄）

Correctly completed application form with photo attached (photos must be 2 inch white background visa-specific ones taken within nearly one year).

3. 护照个人信息页、有效签证页、最近一次入境盖章页的复印件

Photocopies of the ID-info page of your passport, the currently valid visa page, and the page with the latest entry seal.

4. 学费、保险费支付凭证

Receipts of tuition fee & medical insurance.

5. 如离开过常州，需提供新的临时住宿登记表

A photocopy of the new Registration Form of Temporary Residence, if you have ever left Changzhou.

6. 学校出具的委托函

A letter of entrustment issued by JSUT.

7. JW202表的原件及复印件

Original and photocopy of JW202 form.

如果你是新生，除了以上材料外，你还需要持有：

If you are a new student, in addition to the above materials, you also need to hold:

1. 符合要求的照片

取得照片，刻录光盘，以及带有条形码的检测回执（打印店名为“金凯”，在北苑商业街）：

Go to the school print shop in North Garden, take photos which can meet the requirements, obtain photos, burn CDs, and receive inspection receipts with barcodes. (The name of the printing shop is JīnKǎi at Beiyuan commercial street):

2. 由出入境检验检疫局出具的健康证明

The Health Certificate issued by Beijing International Travel Healthcare Center

3. JW202表、正式录取通知书的原件及复印件

Original and photocopy of JW202 form and official admission notice.

You must sort out all the above materials in order. Then you may request the teacher to have a look whether they are complete if you are not sure.
